How accurate is natural family planning? Used correctly and consistently, a robust method like the symptothermal method is highly effective at preventing pregnancy, with peer-reviewed research reporting fewer than one unintended pregnancy per 100 women over a year of use. How effective is natural family planning at preventing pregnancy?
The strongest evidence comes from a large German study of the symptothermal method, which combines two fertility signs, basal body temperature and cervical mucus. Frank-Herrmann and colleagues, publishing in Human Reproduction in 2007, followed 900 women and found an unintended pregnancy rate of about 0.4 per 100 women over 13 cycles when the method was used correctly, meaning couples avoided unprotected intercourse on fertile days. Across all users, including those who took a chance on a fertile day, the rate was 1.8 per 100 women over the same period.
To put that in context, a correct-use rate under one per 100 sits in the same range as many mainstream contraceptives used perfectly. The important caveat is that this figure describes near-flawless use of a well-taught method, which is not how every couple uses it in real life.
What is the difference between perfect use and typical use?
Perfect use describes what happens when a method is followed exactly as intended, every cycle. Typical use describes what happens in real life, including missed observations, misread signs and the occasional risk taken on a fertile day. Every contraceptive has a gap between the two, but for fertility awareness the gap can be wide, because the method depends on daily observation and shared decisions between partners.
This is why you will see such different numbers quoted for NFP. A 2018 systematic review by Peragallo Urrutia and colleagues in Obstetrics and Gynecology examined 53 studies and found typical-use pregnancy rates that ranged widely across methods, with well-studied methods like Sensiplan, a symptothermal method, and the Marquette Method monitor reporting some of the lowest rates. The reviewers also cautioned that the overall evidence base is small and of low to moderate quality, so any single figure should be read with care.
How does NFP compare to other forms of contraception?
James Trussell's widely cited data, summarised by the Guttmacher Institute, gives first-year figures for the United States. Fertility awareness-based methods as a group show typical-use failure rates from about 2 to 34 per 100 women in the first year, and perfect-use rates from under 1 to 5 per 100, reflecting how much the specific method and teaching matter. For comparison, the combined pill is about 7 per 100 with typical use and under 1 with perfect use, the male condom about 13 typical and 2 perfect, and long-acting methods like the implant and IUDs under 1 for both.
Read evenhandedly, this means a carefully used symptothermal method can rival the pill for effectiveness, while long-acting reversible methods remain the most forgiving of human error. NFP also offers things hormonal methods do not, such as freedom from hormones and a clearer picture of your own cycle, and it asks more of you in daily commitment. There is no single best choice, only the one that fits your body, values and circumstances.
Why do accuracy figures vary so much?

Modern sign-based methods are not the old rhythm method.
